Trinity Health Home Care Coordinator in Alexandria, Louisiana

F ull Time, Monday-Friday (Day Shift)

Purpose:

As our amazing Home Care Coordinator you'll be the driving force behind our Nursing team ensuring coordination and supervise nursing services provided to the participants in the PACE Organization (PO). Services may be delivered in the day center or in participants home.

What You'll Do:

  • Responsible for supervision of center and/or in-home staff, including orientation, competency assessment, training, development, and performance appraisal.

  • Coordinate participant care with outside contracted service providers, including hospitals, nursing facilities, assisted living facilities, lab, oxygen, etc.

  • Facilitate integration of new participants into the health care delivery system, including medication, immunizations, routine monitoring of chronic problems, and nursing care plan development.

  • Actively participates in the coordination of efforts towards survey preparedness. When necessary contribute to the plan of correction for department deficiencies.

  • Responsible for adherence to all regulations, both federal and state, which govern the operations of a PACE organization and a licensed adult day care center.

  • Review participant medical records to ensure timely and accurate staff documentation. Responsible for the management of infection surveillance, prevention and control in conjunction with the health system Infection Control Officer.

  • Communicate, counsel and guide participant and families regarding service needs and concerns; make referrals as appropriate.

Minimum Qualifications:

  • Must possess a current, unencumbered active license to practice as a Registered Nurse in the state of PACE Organization.

  • Bachelor’s degree in nursing (BSN) required.

  • Active CPR certification required.

  • Knowledge in home health, hospice, or Medicaid Waiver (Private Duty) experience, required.

  • Minimum of three (3) years' health care experience with emphasis in geriatrics.

  • Minimum of one (1) year documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.

  • Previous supervisory experience required.

  • Valid driver's license and proof of insurance.
