Duties and responsibilities, as required by business necessity may be added, deleted or changed at any time at the discretion of management, formally or informally, either verbally or in writing. Schedule and shift assignments and work location may be changed at any time, as required by business necessity. 1. Youth referrals: Receive referral calls and emails for shelter. Keep track of all admissions and dismissals in order to keep up with current bed availability. Make arrangements to reduce residents when over capacity. 2. Begin working with the youth in law enforcements custody and the family upon the youths admission into the shelter. Specifically, engage in prevention of long-term-out of home care. 3. Promote cohesiveness between Youth Services and other community professionals. 4. Attend court hearings for youth in law enforcement custody. 5. Collaborate with DCF, TFI and St. Francis in implementing case plans. Consistently keep the case workers apprised of all current events with the youth, set up phone and visitation, and coordinate pass privileges. 6. Communicate and work as a team in a professional, respectful, positive and constructive manner. 7. Provide case management services for all Reno County CINCs placed in Detention. 8. Maintain daily case management logs on all youth in Shelter. 9. Explain the court process and standards of behavior to youth to elicit acceptable behaviors while in court. 10. Counsel youth concerning situations affecting their success in the facility program and the consequences for their failure. 11. Serve as youth advocate when communicating with the District Attorney, case managers, and the schools. 12. Oversee youth files to ensure documentation is completed and accurate. 13. Communicate youths case plan information to facility staff. 14. Work with the youth to complete a strength and needs assessment within 24 hours of admission. 15. Work with youth on developing a program plan within 14 days. 16. Update program plans at least every 30 days, or as goals are completed. 17. Report youth behavior progress and special incidents on a weekly basis and other times as needed.
