The centers provide campus-based direct services and supports at 13 centers located throughout the state. This position gathers information to assess the individual's support systems and service needs, including participation in interdisciplinary team meetings, as needed. This position completes an initial social history, as well as maintains appropriate updates. This position may also support the assessment of the individual's rights and capacity to make decisions and provides individual and group training to promote the exercise of rights. The Social Worker II works under moderate supervision, with limited latitude for the use of initiative and independent judgment.Essential Job FunctionsAttends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned. May serve in an on-call status on a rotating schedule as required to meet operational needs of the facility.Interviews and converses with individuals, legally authorized representatives, actively involved persons and agencies to determine what is important to the individual and gather background information needed to assess the individualand#146;s support systems and identify the individualand#146;s psychosocial, economic and physical needs, with updates as appropriate.
