BRIDGEWAY TO INDEPENDENCE, INC IN-HOME SUPPORT / JOB COACH: HYBRID POSITION in West St. Paul, Minnesota

IN-HOME SUPPORT / JOB COACH This position is a hybrid position, meaning there is field work, which involves working in-person with BridgeWay's clients in their homes and in the community, as well as remote work, which will be performed from your home. The In-Home Support / Job Coach provides one-on-one instruction and support to individuals with disabilities to assist them in living in their own home or working towards moving from their family s home and/or to achieve their career goals. A Coach is responsible for evaluating a person s skill set and assisting them in reaching their fullest potential within their career of choice. An In-Home Support / Job Coach with BridgeWay To Independence is employed to help individuals prepare for employment, search for employment, learn, accommodate, and perform their work duties. In addition to working on skills related to performing specific job tasks, the Coach also helps with interpersonal skills necessary in the workplace and during the interview process. A Coach advocates for their clients rights and secures workplace accommodations when needed. The Coach role-models activities of daily living including, but not limited to, morning and evening routines, personal hygiene, cleaning, laundry, daily chores, meal planning, cooking, budgeting, shopping, healthy nutrition, social interactions, community outings, and use of public transportation. Primary instruction takes place in the individual s home and local community. A Coach must be a mentor eager to support individuals of all backgrounds. They must be able to discover people s strengths and weaknesses and help them determine what type of employment is best for them. Having communication skills partnered with critical thinking is a prerequisite for this job. An In-Home Support / Job Coach understands and assists in meeting the mission and service goals of BridgeWay To Independence, Inc., works within the limitations of the agency s resources, and performs all duties in compliance with agency standards. ESSENTIAL JOB DUTIES AND RESPONSIBILITIES Provides Person-Centered Supports: 1. Works with the supervisor and program participants to develop a person-centered plan to assist clients in reaching their highest level of independent living and employment goals. Provides support to develop and follow a consistent training schedule of independent living skills with clients as determined by their support plan. Promotes and teaches self-advocacy, personal, domestic, medical, financial, social/recreational, communication, and behavior management skills to maintain independence. Completes daily written documentation of activities as assigned, including plans, progress notes and reports. Creates a resume that is easy to read, consistent, neatly formatted, showcases work history, and is professional and concise. Prepares clients for interviews using simulations and attends interviews with clients as needed. Engages in effective job search techniques and creates and manages multiple accounts on job search sites. Assesses the strengths and weaknesses of clients and helps to enhance positive attributes. Works to explore client interest in employment by completing interest and personality inventories with clients. Works with the client and advocates as necessary on their behalf to their employer to ensure that they receive reasonable accommodations to address client needs. Completes daily written documentation of activities as assigned, including plans, progress notes and reports. Safeguards client privacy and confidentiality, making sure protected health information is secured in accordance with agency guidelines and state and federal regulations. Works with clients to build skills and increase independence. 2. Works to build client independence in job skills to create as much self-reliance as possible and in an effort for the client to be successful in their position. 3. Works independently and effectively manages time with little to no direct daily supervision. 4. Develops a trusting relationship with the client while maintaining appropriate, professional boundaries while providing services and prompt communication (within 24-hours) to clients and their team members. 5. Maintains consistent contact with the client s employer. Is proactive in addressing employment issues by maintaining consistent contact with the client, employer, and applicable team members. Ensures that the client maintains their employment by keeping open communication with client and employer and meeting or exceeding expectations for frequency of contact. 6. Communicates with clients at least weekly and within twenty-four (24) hours of contact while maintaining professional boundaries. Uses client s preferred communication method. Ensures that they are supporting within the scope of their role and building client independence. 7. Maintains an open and professional line of communication with clients case managers. Follows agency procedures for keeping the supervisor informed of communication with case managers. Keeps the supervisor informed by keeping them copied on email communications and updating them on other communications within twenty-four (24) hours. Communications with case manager are grammatically correct and free of spelling errors. 8. Creates professional, thoroughly written, grammatically correct, and spelling-error-free reports per agency guidelines. Provides monthly updates to the supervisor and sends approved monthly updates to the client and their applicable team members within expected timeframes. Reports require few edits for approval. 9. Ensures client electronic and paper files are secure and documents containing PHI are properly disposed of (i.e., shredded) when no longer up-to-date or necessary in accordance with agency policy and state and federal regulations. 10. Schedules forty-five (45) day review meetings before deadlines and includes all applicable team members. Completes Employment Support & Outcome Methods documentation to high quality standards. Sends documentation to the supervisor by expected timeframes for review. 11. Attends all staff meetings, trainings, and any other mandatory meetings. 12. Other duties as assigned. DRIVING REQUIREMENTS The Job Coach/In-Home Coach position is an unlimited driving position. Employees in this driving category are expected to drive up to thirty (30) miles to and between clients and to transport clients up to fifty (50) miles for goal-related or work activities. Employees are expected to maintain a valid driver s license, a safe vehicle, and auto liability insurance.
